I, the undersigned participant, hereby acknowledge that I am voluntarily participating in Brazilian Jiu Jitsu training and related activities (hereinafter referred to as "BJJ") at Cleveland Institute Of Brazilian Jiu Jitsu (hereinafter referred to as the "Academy"). I understand that participation in BJJ involves physical contact, potential risk of injury, and other inherent risks.In consideration for being allowed to participate in BJJ activities at the Academy, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby waive, release, discharge, and covenant not to sue the Academy, its instructors, employees, agents, and representatives (collectively referred to as "Releasees") from any and all liability, claims, demands, actions, or causes of action whatsoever, arising out of or related to any loss, damage, or injury, including but not limited to personal injury, death, or property damage, that may be sustained by me while participating in BJJ at the Academy.I further agree to indemnify, defend, and hold harmless the Releasees against any and all claims, demands, actions, or causes of action brought by third parties resulting from my participation in BJJ at the Academy.I understand and agree that this waiver and release of liability is intended to be as broad and inclusive as permitted by the laws of the state/country in which the Academy is located and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
I hereby represent and warrant that I am in good health and physical condition and have no medical condition that would prevent my full participation in BJJ activities at the Academy. I agree to notify the Academy of any changes in my health or medical condition that may affect my ability to participate in BJJ safely.In the event of injury or illness while participating in BJJ activities at the Academy, I authorize the Academy's staff to seek medical treatment and care on my behalf, including emergency medical treatment, if necessary. I understand that I am responsible for any medical expenses incurred on my behalf.
I grant the Academy permission to use my likeness in photographs, videos, or other media for promotional and marketing purposes without compensation.I have read this waiver and release of liability, fully understand its terms, and sign it voluntarily and of my own free will.
